Because a small dose and low plasma level found when topical preparations are used, locally applied products on skin do not have a significant excretion into breast milk. Less than 5% of the drug actually reach the blood plasma. On the other hand, clindamycin is excreted in non-significant level intro breast milk with only few cases of enterocolitis in breastfed infants who cured spontaneously after discontinuation of drug. Do not apply it on the breast or cleanse it thoroughly before nursing. The American Academy of Pediatrics rates it as compatible with breastfeeding.
