Proteolytic enzyme which is derived from the fermentation of Clostridium histolyticum that is capable of hydrolyzing and breaking down the peptide collagen bonds. It is a polypeptide formed by about 1,000 amino acids which is a part of the proteases.It is used topically on ulcers and burns and is injected locally in case of localized connective tissue contracture (Dupuytren). At latest update relevant published data on excretion into breast milk were not found. The small dose and low plasma uptake of most topical dermatological preparations make it unlikely a significant excretion into breastmilk.Following local injections on Dupuytren treatment, no measurable plasma levels are detected (EMA 2016: 5.2).Its high molecular weight would hinder any excretion into breastmilk.Because of its protein nature, when orally taken it will be digested in the infant’s gut which should prevent its absorption and passage to the plasma. It has been used on the nipple to treat and prevent pain and various nipple problems without conclusive results. The best prevention and treatment of problems on the nipple is an adequate latching-on technique (Kuşcu 2002, Morland-Schultz 2005). Whenever applied to the chest or nipple, it should be cleaned up before the next breast feeding. |
